Mental Health Case Manager (PROMISE) (CV2195-1) - Reentry Services Division


Mental Health Case Manager 

Provide care. Build plans. Support transformation.

Volunteers of America Delaware Valley is seeking a compassionate and clinically grounded Mental Health Case Manager to join our Behavioral Health & Reentry Services team. In this role, you will support individuals with mental health needs and justice-involved backgrounds through structured, person-centered care planning, risk assessment, and behavioral health intervention.

You’ll be part of a treatment team delivering specialized support that fosters recovery, accountability, and successful reintegration.

What You’ll Do:

  • Complete clinical assessments and create individualized, client-driven service plans
  • Deliver mental health interventions targeting risk/needs for special populations
  • Facilitate group programs, core curriculum workshops, and case management sessions
  • Monitor client progress and adjust care plans in collaboration with the treatment team
  • Assist clients with accessing housing, health care, and other community resources
  • Facilitate house meetings and support reentry-related court and parole appearances
  • Document all case activities and maintain records in accordance with funder guidelines
  • Provide drug and alcohol screenings and support overall program compliance
  • Participate in treatment team and training meetings; support peer staff functions

What You Need:

  • Master’s degree (MSW preferred); counseling or related human services degree considered
  • Minimum of 2 years of experience in mental health counseling with special needs populations
  • Strong communication, counseling, and group facilitation skills
  • Understanding of addictive behaviors and behavioral health treatment models
  • Core curriculum facilitation certification (or willingness to obtain)
  • Valid driver’s license and ability to meet agency driving standards
  • Ability to work flexible hours, including some evenings and weekends
  • Basic computer proficiency
  • ​Must be able to pass criminal background check
  • Bilingual fluency is a plus
